Due to the World Social Work Day 2019, we would like to address those readers from other countries who follow our blog. We want to thank you for your interest and try to explain who we are, where we are and what we do.
Our story started 4 years ago, when we thought how interesting it would be to show what medical social workers (TSS) do at the Catalan Institute of health (ICS). So, we decided to create this blog.
Let’s explain how everything started:
The Catalan health care model has a diversity of health care providers who manage health services for the public and are contracted and certified by CatSalut.
The system of health: It is universal and to access it is necessary to identify with the individual sanitary health (TSI).
ICS is the biggest healthcare provider in Catalonia. We are more than 41.000 employees working for this public company. Our head-office is in Barcelona, but we have health centers all over Catalonia. healthcare activity 2017
Medical Social Work has been growing in ICS since the beginning. We are currently 280 people working, 240 of which are medical social workers who work in primary care centers, 40 work in hospitals. In Catalonia, everybody has a Primary Care Center (CAP) assigned to where you can go if you’re not feeling well, or in case you want to start some preventive health treatments. In every center there is a primary care team (EAP) formed by doctors, pediatricians, nurses, odontologists, medical social workers and administrative assistants. We are part of the team and we share the medical record with doctors and nurses.
We use a electronic health record (ECAP) to register all the clinical information and use it to have the most accurate information of the patients. We register scales and activities we have done so the software can tell us which patients should be visited because of illness or social risk. That software is in all the ICS centers and many other providers, so we can see all the information from every place in Catalonia. ECAP gives us the opportunity to go in depth in the analysis of data, so we can focus in proactive and pre-emptive work.
All medical social workers are working as a network: at hospitals, medical social workers are trying to refer patients who are getting illnesses that will cause social impact to CAP’s TSS.

We have different activities and continuous learning trainings we have for ourselves, or also for the community, because we work and collaborate with many organizations (municipal social services, associations, third sector entities…). Also, you can find information about how we develop our job, how we work to improve our job tools we use to analyze social risk, etc.
